if you mean by "3 tomoe" a normal sharingan then you have to know that the normal sharingan and the mangekyou sharingan's functionality are completely different. and it depends on the induvidual (that's why Itachi's and kakashi's are different)
mangekyou sharingan been more powerful than a normal sharingan i'd say it probably can trick the other.
but it will also depend on the experiance the user have on the sharingan. e.g: Sasuke only saw, identified and understood the kyubi in Naruto only in Shippuden after almost 3 years he aquired the full sharingan. He only saw the 9 tail's fox shaped chakra the first time he aquired the full sharingan.
also Itachi anhielated the entier Uchiha clan using his mangekyou sharingan (although we don't have much details on it). so it probably can.
and when Itachi used genjutsu on kakashi, he couldn't get off of it even though he knew it's genjutsu. (but i can't remember if itachi used his mangekyou sharingan on kakashi or not)
so I guess there is a possibility that mangekyou sharingan can compleately fool the normal one.

Your question sounds a lot like episode 83 or 84. Itachi, a mangekyou sharingan user, used a genjutsu on Kakashi, a 3 tomoe user. Kakashi knew it was a genjutsu, but he couldn't escape it because it was so powerful.
it depends. sometimes yes and sometimes no.depends on whos using the mangekyo sharningan like Itachi or Kakashi then no one would no until....KABLOOOIE! they've got their body sucked into some other dimension caused by the sharningan itself.your welcome.
wow that's a really, really confusing question, does your question come from that episode when Itachi and that Shark guy attack Konoha?
It depends on the genjutsu . .
so far, the only genjutsu casted by itachi are obviously alternate dimension genjutsu . . .
