No, not that i have seen.
No, you never get to see the whole thing. But there are many moments where you get close. Like in Shipuuden episode 33, he is not wearing his mask, but has bedsheets covering up his nose and mouth, and the sheets slip down a little.
And there was a Naruto Shipuuden cover a little while ago where he wasn't wearing his mask but had his face covered up in one way or another...I think he was covering it up his book, but I can't remember, sorry.

Not that I know of, there was a fake picture of Kakashi without his mask in circulation a few months ago.
But I don't believe that there have been images with him without his mask that are authentic.
