As far as I'm aware, they've never detailed how the first two Hokages died.
Madara is apparently Tobi, though that is a bit suspect considering how old Madara must be. Tobi at least claims to be Madara and has a Sharingan eye. After an argument, Madara was defeated by the First Hogake in battle and left the Leaf Village. That's the last solid info on him.
No, Naruto's mom died sometime while he was young, likely before the Fourth sealed the Nine-tail in him.
The first and the second presumably died from protecting the leaf village from attacks. Madara is somewhere but we don't know where. Naruto's mom is presumed to be dead, since tsunade and jiriya were talking in past tense.

i hate to admit it, but with the solid reference " tobi "so far in the manga he's sitting on the statue of Madeira in the valley in the end supposedly watching over konaha. well i believe that gained immortality in some way, like orochimaru
sadly no, his mother most likely died while giving birth or of some sort before the kuuybi was sealed in him

4. Naruto is an orphan, he never met his mom. His mom is believed to be dead.
