Well, if he did lose concentration, his "Spider senses" would warn him of the danger... But that would be funny to watch though.
i think it sucks how Harry's stupid butler knew about his dad's death since it happened and waited so damn long to tell him, and it's stupid how Sandman tries to kill Spiderman throughout the whole film and in the end all he did was say sorry, cry and Peter 4gives him. fighting all that time and all they had to do was talk to end it all.

It has happened before. there have been comic issues where he lost his spider sense due to an illness or being knocked senseless...and then while traveling -- WHAM! Usually into a misjudged building, but I think he did hit a telephone pole once, IIRC.
