According to Marvel, the difference is one is "ultimized", which isn't very helpful. My understanding is the "ultimate" dimension allowed a retelling of the characters without all the baggage of the ones we knew for several decades. Marvel wanted to make the characters more modern and accessible to new readers.

How it began? Well, if you mean the dimension, that question hasn't been yet. I believe Marvel is leading up to an origin of the Ultimate universe.

As far as which is better, it really is a matter of taste. I like the Ultimate Spider-Man much better than the regular Spider-Man. Everyone raves about the Ultimates, an ultimate Avengers, and you can read it without the years of delays.

The difference between the Ultimate Marvel Universe and the regular standard 616 Marvel Universe is that in the Ultimate one everyone has been redone to make them more easy to understand by newer, younger readers who may not want to go through 60 years of X-Men history and etc.

The characters were often made younger, most had some personality changes(most seen in the Ultimates line with Captain America and some others) and storylines redone.

I only get the Ultimates so I couldn't say which was better. It would depend on how long you've read the regular versions.

As for a list, you could try wikipedia's Marvel database. They should have one.
