Yes, the whole point of Itachi telling Sasuke about the story with Madara was that you have to take your brother's eye to obtain the permanent mangekyou sharingan. Kakashi is not related to either Madara or Itachi, so it wouldn't matter what Kakashi has.
Yes, in the story of Madara and his brother, Itachi said that both of them had evolved the Mangekyou, but since Sasuke still doesn't have the mangekyou and Itachi still wants his eyes, I guess that your sibling does not have to have the mangekyou as well. Itachi just wants Sasuke's eyes because Itachi is going blind, and taking Sasuke's eyes will enable him to never lose the Kaleidoscope's light. As long as Sasuke has Sharingan, Itachi will be fine, because you don't need another mangekyo, as Itachi already has it.

That is possible, but what it most likely means is that in order to reverse the negative effects of the original mangekyou eye, one must double the sharingan's power with another sharingan. then again, if this was the case, itachi probably would have taken sasuke's eyes a long time ago, or at least, before sasuke went crawling to orochimaru. So ur assumption is probably correct that the sibling must have the mangekyou sharingan as well. as for kakashi, his case is a bit different since it's actually obito's eye, and not the one he was born with. so, that pretty much means that kakashi dosen't have the permanent mangekyou, since he has not uchiha brother who's eyes he can take. hope this helps a little bit, even though it is lengthy. good question though.
well if itachi does get sasuke eye he might have to activated again or something .lol he is going to kill the kisame....
