In the case of the three renegade captains and 13th squad Lt Kaien Shiba, the posts have not been filled so I have listed the previous holder of said post.
For 13th squad Lt, the third seat was Kaien's wife who died just before him which may explain why there has not been an Lt for this squad since the start of the series, especially as Byakuya will not allow his little sister Rukia to take any position that puts her at risk of getting the more dangerous missions. Since then the role has been filled unofficially, to good comic effect, by Kiyone Kotetsu and Sentaro Kotsubaki who are always trying to out do each other in order to get the role for themselves.
