Naruto's family name Uzumaki was taken from his mother Uzumaki Kushina.
Uzumaki Arashi was a "rumored" name for Yondaime; that was before his real name - Namikaze Minato - was announced in the manga. I think fans are saying that Yondaime must be an Uzumaki because Naruto's family name is Uzumaki (normally children take their father's family name, right?).

Well there are two scenes in which it happens in. The first one is in chapter 367. It is first disused between Jiraiya and Tsunade,just before Jiraiya was about to leave. Later on in chapter 370 we find out his true name which is Minato Namikaze. Later on in chapter 382 we actually see Naruto's mother and father.
We actually find out who's Naruto's mother is. She is Kushina Uzumaki (うずまき クシナ, Uzumaki Kushina?) is a ninja of the former Land of the Whirlpool.
