You can never actually see his face, but Episode 101 is a filler in which Team 7 tries to get him to show his face, only to reveal that he wears another mask underneath.
It doesn't show in the manga..but it's just a normal face under his mask, it's not bruised or anything
They have never show it. They have not show it yet in the episodes or the manga. Although the only episode that it came close to that it is episode 101. Where Naruto,Sakura,and Sasuke try and find out whats under his mask,they try different methods throughout the episode,but the fail. So,at the end they just ask what is under his mask,he takes it off and it is revealed to be another mask (lol).
Most of the time Kakashi face is always hidden by something,like a book,a towel. etc So,so far we really don't know what he looks like. Some people say the only reason why he wears a mask is because he looks like his father the white fang's and doesn't want to get mistaken for him.
If you see any pictures of him without his mask on it is just fanart,not real.

The simple answer to both questions is nowhere because Kakashi always wears a mask in both the manga and the anime. However there are various fanart pictures of Kakashi without a mask but they are all just speculation and the artists imagination.

In this episode based on a special omake, Naruto convinces Sakura and Sasuke to help him see what Kakashi's face looks like under his mask. Each one of their plans, such as buying him ramen to force him to remove his mask while he eats, fails in a comedic fashion. At the same time, they inadvertently and unknowingly thwart attempts by three ninja to kill Kakashi. Finally, Kakashi agrees to take off his mask, revealing another slightly paler mask beneath it.
