he's gonna restart the uchiha clan with sakura stating winth 1 little uchiha, then 2, then 3, then 4, then 5, then 6, then 7....
He will go after Uchicha madara. since, sasuke already knows madara is itachi's accomplice in murdering all uchiha clan. he'd definitely go after uchiha whether he can't beat him or not. and there is no guarantee yet that itachi is totally defeated. i think he just lost the strength to even move. casting that susano-o jutsu is just too much a drain for even the likes of him. and there is still a threat for another orochimaru respawn. remember that, ororchimaru's state of mind is inside kabuto. so orochimaru might be seen again.
so, if he successully kills madara, he'll finish his business with orochimaru, and then itachi again, and then he decides if he'll return to konoha. that is.. if konoha wants him back.

We don't know yet if he realy killed Itachi. And even if he did kill Itachi probably he will wonder around the world, Naruto will chase him....they will probably meet again, maby they will defeat Orochimaru together, but in the end I think that unfortunatelly Sasuke won't go back to the village.
