If I remember correctly it was in battle with the Shadow King originally, then the same thing happened in his cloned body around Uncanny X-Men #280 / X-Factor V1 #70.

Most people forget about the cloned body.

Would you believe Lucifer? Originally, an alien calling himself Lucifer dropped a stone block on Professor X's legs. He was "permanently" crippled until he was cloned. I stopped reading X-men so I don't know what happened after that.
