The music box symbolizes Lucy
And the clock represents Nyu!.
Lucy is killed off during the scene after the kiss!.-she goes back to the bridge, where more soldiers are waiting!.
(The music box melody stops- Lucy dies)
The clock (Nyu) starts working when you see the shadow at the door!. also pay attention to how the shadow stands- the knees are bent inwards symbolizing Nyu!.

Well in my mind --> I always imagined that that person was Lucy!. I felt that she had come back to visit Kouta once again after finally being rid of her horns and her past!.
Darn, I was wishing there would be a sequel to Elfen Lied that would pick up after the clock starting gonging and would show who the shadow behind the door was!
