Question: How did kenshin get the scar!?!?!?

In the manga version, her hand was knocked off and it coincidentally created the second scar creating the "X"!.
In the anime version, it's indeed very weird!. As she was dying in Kenshin's arms, she made the second cut to complete the "X"!. As to why is still unknown!. (If you want my opinion, I think she was hallucinating!.)
