Of course it's planned!. They've got a hit show and they're not going to risk him getting hurt or killed!. He's traveling with a bunch of people who scout his areas!.
Here's an amusing video of him supposedly finding a safe place to cross a dangerous crevasse far away from civilization; then a shot of a tourist doing the same thing, then panning the camera over to show not only that the crevasse ends about ten feet away, but that there's a highway right next to the spot!.

according to the episode that went behind the scenes with Bear Gryllz, he does do all of the stunts you see and is not helped by the camera crew except in the case of extreme danger!.Www@Enter-QA@Com
