Yes, it's all real. The cameramen/man is there to document only, not assist. (Although I'm sure in a true emergency he's allowed to.)
IMO, not all of what Bear does is necessary. Eating bugs or maggots makes for good TV, I guess. I've seen him do it often when there were other sources of food on camera.
