You need to pick up the handkerchief that she's dropped (I take it you're playing Shivering Isles!?) and go and see the bone archer man!.
He'll take you to a garden that has a few skeletal warriors and the corpse of a gatekeeper - he'll make arrows (should he survive the fight) from the corpse's bones!.
Poisoning these arrows with the vials of poison made from the handkerchief (really, check the floor where they spoke around midnight if you can't see it) and using them on a GateKeeper Bone Arrow - you can kill him in three shots (but I was a master marksman!.!.!.)!.
Then you just have to make the choice - Mania or Demetia!.!.!.

Yeah like saccade says except you have to talk to the sorceresses assistant who will tell you about the tears, it's been a while since I did it so I can't remember her name it's something Don i I think, I don't think you need the bone arrows if you have the tear stained handkerchief, I think you can just poison your sword with it!. but either way it isn't too difficult Www@Enter-QA@Com
