I would like to give you some helpful advice.
But your comment "mars is square mars" stumps me.
Is it a typo?

NB If you have one planet square to Mars & another in opposition, you have a classic T-square. This T-square would cause you a lot of frustration, and you would tend to "release" this frustrated energy through the sign and house opposite the planet square your natal Mars.

If you provide some more information on what planets they are, and what degrees they are in, I would be happy to offer you some specific advice.

Mars is in triplicity in Pisces, which suggests that Mars in Pisces is a "lucky" planet for most people <triplicity is a type of essential dignity>. By lucky I mean "being in the right place to take action at the right time". The luck comes from good timing more than anything else, rather than talent or ability.

Well Mars is the God of War so it is naturaly a fighting and angerish sign. Pisces are very wise and smart but yes it could bring out a nasty drinking or durg side of people.

It depends if mars is placed at the beginning of the sign which expresses more on the mental side, or towards the end of the side which makes him more physical.

As for Pisces/Mars, his energy is lessened and the person is more reserved in his actions. But remember that it doesn't mean you have no energy. Mars still gives power but isn't apparent on the surface. People know how to bide his time, and to choose the right moment for action.
Also, the sensitivity and feelings are very strong here. It's not the best place for Mars but not the worse either.

Now, you didn't mention any aspect or house position. Aspect reveal the good or the bad outcome and in what areas of life by the house position.

Many policeman and people who work in prisons as guards have Mars in Pisces. Others, work in petroleum , foundries or in pits underground.

As for the drugs and alcohol issues. NO. He would have to recevie bad aspects from Neptune to be in that position. And the 12th house would have to contain either of these planets especially Neptune.
