I've been subscribed to NG magazine since last April, so my subscription has ended [I think].
If I want to renew it, do I send a new subcription form to them or do they send you something to renew it?

What was the date on your first magazine? Even if you received it in April, it may not have been the April issue. Did you subscribe for more than one year? Look on the label of your last magazine (or any of them). Your expiration date should appear on the label.
You should have received a renewal long before now. Magazines usually start 6 months prior to expiration. Unless you subscribed from an agent...then sometimes the agent should be sending the renewal.
Go to www.nationalgeographic.com and click on Customer Service on the left side under the Subscriber button (do not click on Subscribe - you will create a duplicate subscription and this is a hassle you do not want). You can see the status of your current subscription by entering the information they are asking for.
