I don't know if you consider this "mainstream," but a 1915 film called Inspiration is considered the first American film to feature nudity. There were actually a lot of American films to feature nudity before Hayes Code was instituted in 1934. The first film under the Hayes Code to have nudity was The Pawnbroker (excellent movie, by the way). I would certainly consider this 1964 film "mainstream," as it was nominated for an Academy Award.
