When Beth later sees Sara, she says not to trust Juno, tells her she left her to die, and shows her the neclace. Later when Juno meets up with Sara and they are being attacked by ALOT of those things, Sara pulls out the neclace. At this point, Juno knows Sara knows about BOTH secrets. Sara wounds Juno with the ax in the leg and leaves her to die. Whew!

It's been awhile since I saw the movie but I think that Juno may have been having an affair with her girlfriend Sarah's deceased husband Paul, thus explaining the inscription on Juno's necklace. The cave expedition is led by Juno, a thrill-seeking, headstrong and irresponsible woman. She intends the trip as a way of making it up to Sarah after running out on her when she lost her whole family – husband and young daughter - in a brutal car accident. But for sure one of the main secrets is the fact that Juno has no real concern for anyone (but resolving her own guilt) so she neglects to tell her friends which cave system they are really caving in, neglects to bring the guidebooks, and neglects to file their trip with the appropriate park authorities--all in the hope that discovering an uncharted cave system would make amends.
P.S.--Another "secret" of Juno's is that she inadvertently wounds one of her comrades (Beth) while in the cave but doesn't confess to the crime. Beth lives long enough to tell Sarah what happened and reveals the necklace just before she dies.
