Irreversible. There is a scene where some guy gets his head smashed in with a fire extinguisher. And it's the realest thing i've ever seen on film. There is also a 9 minute rape scene in the movie. It's not a work of art for the faint of heart. But... they did manage to escape it being a snuff film, it's a very hard to digest piece of magnificent film.
even we watch a lot of violent movies in thearter, DVD .....but have anyone watch all of them? i mean around the wordl ! more and more movies come out which are more violent so just wait and see ....or may be depend on people think which one is most violent thou. However, the realistic movie from the Iraq war or the scene of 9/11 are the most brutal moments rite! =D
Like the above answer said Irreversible (1992) definately has too of the most brutal violent scenes I have ever seen in a movie and I've seen alot.

Man Bites Dog (1992 ) is a true work of genius.
Another violent comedy, but you must see the UNEDITED version of this to truely see how twisted and depraved the antihero is.
Ichi the Killer (2001) is a movie I have on disc, but have never been able to get past the first ten minutes of.

The Wild Bunch is a good violent movie ,,it was made way ahead of it's time...
