Mark leaves Vanessa. Juno has her baby and decides to give it to Vanessa anyway since she knows Vanessa will make a great mother.
**SPOILERS TO FOLLOW**
Juno doesn't keep the baby. She leaves a note for Vanessa (who is going to divorce Mark), a not which reads "If you're still in then I'm still in", meaning if Vanessa is still willing to adopt, Juno is willing to give up her baby.
Juno does give up the baby. Vanessa is shown holding the baby lovingly but very nervously, and Juno's stepmother says she looks just like a new mother... scared ****less but happy.
Juno tells Paulie that she loves him at some point in the movie (don't know if you caught that part) and in the end they're "boyfriend/girlfriend" and very happy together. They meet at the very end and sing together. They sing the song that played with the opening credits: "You're a part time lover and a full time friend / The monkey on your back is the latest trend", etc.
