It can cost tens of millions of dollars to put copies of a film into 3,000 theaters and support them with nationwide TV and newspaper ad campaigns. Only the big studios can afford to do this, and only on their most sure-fire commercial movies (like, say, "I Am Legend").
For an independent movie like "Juno," the prints-and-advertising cost, even for a handful of theaters, can run more than the film cost to make. So indie distributors (or the sorta-indie divisions of big studios, like the one behind "Juno") release their movie in a few theaters in carefully selected markets, where they hope the film will generate good reviews and positive word-of-mouth, and expand itinto more theaters in more cities if the film does well.
Releasing a movie like "Juno" at the end of the year, when awards voters are likely to notice, helps as well. You can expect the movie to open in more theaters if it picks up a few trophies at the Golden Globes or the Oscars.

They are releasing it only in select theatres right now hoping to build good word of mouth as well as Oscar momentum. This weekend it will be showing in just 994 theatres but it will most likely be expanded to many more theatres after the new year.
