In the earlier parts of the Movie, Alex often fantasized about violence, usually while listening to Beethoven and especially while in prison when he was reading the Bible.
After the "treatment", he can't even think about sex, violence, or even Beethoven without becoming violently ill. When he is victimized by an old victim, he tries to kill himself. He is then "cured" from the affects of the "treatment" he received. After he is "cured", he goes back to his old fantasies.

The book has two endings. Well, there's the real ending - a final chapter in which Alex sees one of his droogs who's settled down into a normal life, with a wife and kid, and he resolves to give up the ultra-violence and become a decent person of his own free will. That chapter was left out of the American version, instead ending with Alex's return to violence without the redemption thereafter...

It's an ironic statement, at the end of the film, saying he was "cured." The fall from the building reversed the Ludovico Technique - notice that the music no longer makes him ill - and it also made the government want to use him for PR purposes. Basically - he'll be free to return to his ways, this time with the tacit approval of the state.
