the ones one pirates of the caribbean 2 are the funniest, but the ones from the other two are almost just as funny

The ones at the end of the Shaun of the Dead DVD are the only ones that come to mind instantly.
Hot Fuzz has some good outtakes so does Lord of the rings (Gandalph in a wig with pink ribbons lol) a lot of dvds have blooper reels, and if you go on youtube theres loads

A famous one was in the Alfred Hitchcock movie, "North by Northwest." In the scene in the lodge at Mount Rushmore" just before Eva Marie Saint fires the pistol, a little boy covers his ears! Hitchcock must have been furious.

During the scene of the meeting between Moe Green and Michael, The boom mike can be seen clearly, as Moe sits down for the first time. A shadow also reflects a hurried movement behind the camera.

well lets see theirs the guy who hangs himself in the original wizard of oz and their the well known ones in the disney movies like in little mermaid the preist has a bonner at the wedding etc one i noticed is in the santa claus tim allen goes out side to talk to his ex wife the morning after he goes to the north pole when he goes out he has no boots on then when he goes back in he has boots on
i like the one in one of the star wars films where a storm trooper hits his head on the door frame
on the original you can hear a plastic "clack" sound but on the dvd they've added a "thud" sound effect so it sounds like metalish stuff

I know of two that no one has mentioned yet.
In "Teen Wolf" at the end of the movie, there is a part where I guy shows his penis for a couple of seconds.
The other one is in "Rocky Horror Picture Show" Towards the end of the movie, when Riff Raff tells Frankenfurter that it's all over and they are returning to Transelvania, Janet steps on Brads toes and you can see Brad in pain.
I love the Star Wars episode 4 with the storm trooper hits his head.
In the movie Lord of the Rings: Fellowship of the Rings, when the hobbits are with Aragorn on their way to Rivendale, Sam asks Aragorn where they're heading, and Aragorn says into the wild, or something like that, you can clearly see Aragorns sword hit the camera.
Also in Serenity, when Mal and Inara are fighting the Oporative, there's a moment when some tall poll hits the camera. Also later in the movie, when the alliance have the crew surrounded after River kills all the reavors, the Alliance are putting their guns down, and off to the left of the screen, you think that a gun is going down, but it's actually a camera (Joss Whedon talks about it in the commentary on the original dvd)
