Canadians don't look that different at all. It started with "Terrance and Philip", the show within the show that the S.P. kids watch. When South Park debuted in 1997, critics put it down as "the worst animation and nothing but fart jokes." So, the creators decided to show the critics they were wrong by creating T&P who do nothing but fart and don't look like people at all. Somewhere down the road, their strange appearance was attributed to the fact that they are Canadian, just like Kyle's adopted little brother Ike.

I think they were making fun of the fact that Canadians and Americans look (and generally act) exactly the same.
