It would very much be like lice or other parasites known to plague large animals. A lot of parasites secrete chemicals in its saliva to aid in the siphoning of blood/nutrients. A well-known example is mosquito saliva, which contains an anti-coagulant that also causes the itchy allergic reaction you get after a mosquito bite. If a tiny mosquito causes an inflammatory response in a much bigger animal such as ourselves, it could be extrapolated that the effect could be fatal (and graphic) on a creature that is close to the mosquito's size. Do you see what I'm getting at?
