It's up to you, really. She dreamt escaping the cave and then woke up. She later imagined her daughter and the birthday cake. And it's assumed that Sarah died, as you can hear the creatures.
If you look carefully you can see five candles, but then you see six candles, probably symbolising the six women, in the cave, dying.
Also she imagined seeing her daughter, so it could also imply that she may have imagined seeing the creatures and killed her friends herself.
It's mainly to keep you thinking and decide how you think what actually happened.
The lady in the cave was knocked out and imagined the film from then onwards, then she woke up and saw her daugter's birthday cake, and her daughter.
There are 2 endings.
Long story short, she gave up and stayed there accepting her fate.
The cake wasn't really there - it was in her mind's eye.
