It says in the movie that some people say his soul escaped from him... it was in the suitcase that John Travolta and Samuel l Jackson were retrieving..
there's a link on this page also that says there was really nothing in the suitecase and q.t. left it to the imagination of the viewers..

I would imagine it had something to do with the street brawl with Bruce Willis orrrr.... The hillbilly Sodomizing in the pawn shop??? All and all he was a large scary rough brotha whos to say it wasn't a head shaving accident.
My guess would be he had a cyst or boil removed
from the back of his neck. Because a buddy of
mine had to do the same thing.
