There are two common ways to make an actress look pregnant. One is simple padding the other is to use what is called an empathy belly. If they are showing the bare midsection, then they would probably have to have a mold of the midsection and have latex properly fitted.
As for how they determine which baby will be used, like anything else there is a casting call (depending on age) and see which babies are the most photogenic and less likely to cry.
