Yeah, i was confused too when i first saw the movie!.!.!.i think that they deliberately left it as an open ending, depending on how YOU chose to view it, coz there are clues that support both the interpretations(check out its wikipedia article)
I think that he DID kill his friend in the beginning, the rest of the mayhem somehow seems more like he imagined it all in his crazed haze:remember how each murder gets more and more graphic and more improbable!?like how he tries to feed the cat into the ATM!?or how he manages to blow up cops and a car in the street without getting caught!?and somehow always blood cleans up, and in the end even the bodies in the flat disappear!.!.!.!.!.!.!.!.!.!.!.!.!.!.!.!.!.!.!.!.!.!.!.!.!.!.!.!.!.

I think he actually did it! and his world is so full of vacuous people that are full of @***@￡￡ and wrapped up in them selves that they can't even believe or comprehend that something like that would happen in their rich and wealthy lives!. They think he is joking!
