Usually what happens is, especially if there is nudity, the director and producer will meet with the talent and decide the best way to go about the scene!. Often times there is only a bare bones crew there to handle the technical side so the actors can be more comfortable!.
Since they are acting, and only acting, their comfort is very important to the production!.
