How do you respond when somebody wants to shake your hand?!

I ususally just smile and act the fool whilst leaving their hand hanging in mid air...I hate, absolutely detest shaking peoples hand. Think you are a germaphobic! Meet your new neighbour! *Cringes*
i keep my hands in my pocket and just nod.
i bleach my hand after
i scratch my nose or my butt then put my hand out,by then their gone!
i tell them i have carpal tunnel syndrome and i can't.
I shake their hand
i just do.then i wash my hands as soon as i can.

I never shake hands with someone I dont know at all. I mean like someone youve never seen before. Now, If I had an appointment with someone, I didnt know, Id shake their hand when we met. And I always shake my friends hand when we se each other...or give em a hug.*
