I live in Scotland and we say it quite a lot but we say 'are ye a man or a moose?' meaning are you brave or a wimp.
So going by your truth bit, a man would be brave and tell the truth and have guts, but a mouse wouldn't - it would just hide so the man's likened to a mouse coz hes a wimp.
