Yes Scrubs is filmed in a real hospital. It is on the outskirts of north Hollywood. It was abandoned and they fixed it up for the show.They actually use a floor of the hospital for the actor's and writer's and producer's offices.

In the documentary they said that it was a real hospital but it had been abandoned. Apparently some people turned up with broken limbs thinking it was in use!
