I asuume they broke up in the season 6 finale when lex supposedly "killed her".
but as she faked her death to get lex put in jail for murder, she got with clark in season 7 episode 3 (Fierce). and are still together so far this season (7x10 is next episode).

Contrary to what the poster below says, Lana is still alive and well in season 7. They are currently together. Whether she stays alive til the end I don't know.
