All Hell Breaks Loose is the third season finale of The WB drama series, Charmed. This is Shannen Doherty's 66th and final appearance in the series (67th including the unaired pilot). Michael Bailey Smith who appeared as Belthazor in the past, starred as both Shax and The Source in this episode.
Summary
Piper and Prue are protecting a doctor, who is followed by a demon assassin named Shax for having saved too many lives. The doctor can't believe demons exist and thinks all this is a joke... until Shax enters and throws first Prue, then Piper through a wall, shattering it and almost killing the sisters. Luckily, Phoebe arrives, hurts the demon with a spell, scares him away and calls for Leo to heal Piper and Prue.
Phoebe then goes to the Underworld to help Cole fight the evil he holds within, and Leo consults the Elders about whether or not Phoebe vanquished Shax. Prue is suspicious that she did not, and she and Piper walk the streets in an attempt to find and vanquish a weakened Shax. When Shax appears and attacks, Prue deflects one of his attacks, and Piper uses her new power and blasts him, and he disappears again. The sisters run back to the Manor so they won't be seen, unaware that a news reporter shooting a story on their street caught the battle on camera and broadcasted the story live.
The story follows the footage back to the TV station, where it's decided to check out the sisters and air the footage again. It then goes to Darryl at the police station where a superior tells him the rumors about the Halliwells must be true, alluding to the news broadcast seen across the room. Darryl is shocked and immediately phones Prue and Piper about their exposure. The sisters consult Leo, who tells them that the Elders called all Whitelighters away so they won't be exposed as well, and that the Charmed Ones have to srt this out on their own. Keeping in mind that Shax is still alive and might be a danger to the doctor, Prue and Piper find the doctor in the hospital, but when they come out, Shax appears, and Piper and Prue have no other option than reciting the vanquishing spell in front of the hospital and several people. They all applaud when the demon is dead. Once again, the battle is captured by the reporters, who had lojacked the Halliwell's car to follow them.
Phoebe finds Cole in the Underworld, and he attacks her, convinced he is dangerous. She uses a potion she created to purge him of evil, and it works successfully. She decides to stay in the Underworld for a little longer with Cole, unaware of her sisters' exposure in the mortal world. Meanwhile, Leo goes to find her and Cole, leaving Piper and Prue without a Whitelighter (as he can't sense them from the Underworld if they are hurt or in danger).
Both video records are shown on TV and cause a sensation. A large crowd forms in front of the Manor which consist of journalists, believers, curious people, and a girl named Alice who believes herself to be a witch (though she isn't one). While Piper and Prue discuss what they will do about all this, Alice blasts into the house looking to join their coven, but Prue throws her out with her powers. A hysterical Alice claims that Piper and Prue are bad witches to the reporter outside. Alice steals a rifle from the SFPD and shoots Piper fatally in the stomach through the window. Prue tries to take her to the hospital, but the crowd holds her back, forcing Prue to use her powers to throw people out of her way. Piper finally dies in the hospital emergency room under the care of the doctor they saved earlier.
The Source learns about all of this and is in trouble too: by exposing themselves, evil is also exposed. After Cole, Leo and Phoebe learn about Piper's death, they find a risky solution: they have to contact Tempus and have him rewind time. The Source tells them that Tempus is weak now and could die in process. The Source finally accepts, but with some conditions: Phoebe will not be affected, but must remain in the underworld for all time, and time will be rewound to Shax's first attack. Phoebe agrees, as long as Cole is sent to warn her sisters about Shax's attempt to kill them. Both sides agree on these terms. However, the Source has his demons hold Cole, Leo, and Phoebe hostage, and tells his men to kill them once time is rewound, in an attempt to destroy all of the Charmed Ones.
In the hospital, special forces break in. Prue blocks the door to the operating room where Piper's body lies. Just as a bullet is about to strike her, time then rewinds to Shax's initial attack. He manages to throw Prue and Piper through a wall, and because Phoebe is not there to wound him, blasts the doctor across the Dining Room and into the window. His mission successfully completed this time, Shax flies out in a whirlwind and breaks the glass in the doors of the manor.

It was the season finale. There were 3 demons after them...I think. Somethings happened and the whole house blew up and she died. Sorry, I can't remember all the details it was a long time ago.

The why is because she was having problems with the rest of the cast. It wasn't working out at all so she left.
